Can I extract a file from the Setup package using its own Shell Operation?

While the Setup package is running I would like to extract a file from its list of files at startup and have this extracted item placed into TEMP and then launched, all this before the wizard kicks in to install the setup package. Is this possible?

It seems like i would be using the Shell operation, "Execute program" at startup. Perhaps calling something like ExtractCab Setup.exe
