How to make a custom window?

I have to make a cistom window with radio buttons, check boxes, more than 3 buttons.
How can I do that?

Sorry but you cannot create a custom screen with those options in Setup Factory 5.0. Please see the Screens section in the User's Guide for more information on screens, and the available Custom Screen options.

I suggest you go to: http://www.indigorose.com/setup5/update.html and update to the latest version of Setup Factory 5.0.
