Is this possible - registration form after install done.

I want to be able to have the ability, that after my setup program has been executed, all the files copied etc.... that it presents the user with some sort of Registration form which once they complete, can either fax or email back to us.
This will be useful to determine who has the program installed in case of any updates etc. required.
Anyone any ideas on how I should go about this?

In Setup Factory:

1.) Go to screens.
2.) On the top tabs. Choose (After Installing).
3.) On the after installing screen. Add a finished screen by clicking the bottom left button and choose (Finished Screen - Top Banner or Side Banner. If you have done already, skip this!
4.) Still in the After Installing screen. Double click on the finished screen - top banner or side banner.
5.) On the top tabs of the new screen. Choose (After).
6.) On the After screen. Now click the Add button and hold mouse over (Open/Close Programs). When the menu slides out. Choose (Open Document).
7.) When the new screen opens. Stay in Settings. Where it says File to open (path and file name):
