Actually, that would be a relatively easy application to make with AutoPlay Media Studio 4.0 (to be released next week). AMS40 allows you to collect data from the user in edit fields and then submit the information to a Web site. The Web site could then store the information in a database, email it to the developer, etc. The Mad Cubist could probably tell you how to do that kind of thing with AMS40/PHP.
